Informed Consent for Telehealth Services

1. Nature of Telehealth Services

Telehealth involves the use of electronic communications, including secure video conferencing, telephone, asynchronous messaging, and electronic health record platforms, to enable licensed healthcare providers to evaluate, diagnose, and treat patients at a distance. Concierge Med Partners provides telehealth-based weight loss management, GLP-1 therapy (tirzepatide), peptide therapy (including GLO/Glutathione and MOTS-c), NAD+ therapy, and hormone therapy (TRT/HRT) to patients nationwide through its network of licensed providers.

2. Expected Benefits of Telehealth

  • Convenient access to licensed clinical care without the need to travel to a physical office;
  • Faster access to evaluation, prescription management, and follow-up care;
  • Ongoing, responsive communication with your assigned provider through secure messaging;
  • Direct-to-door medication delivery through our licensed pharmacy partners;
  • Personalized, medically supervised wellness protocols tailored to your individual health profile.

3. Potential Risks and Limitations of Telehealth

As with any form of healthcare delivery, telehealth has inherent limitations. You understand and accept that:

  • Information transmitted electronically may be insufficient for your provider to make certain clinical determinations that would be possible in an in-person setting;
  • Technical failures, including internet outages or platform disruptions, may interrupt or delay your consultation or communications;
  • In rare instances, despite our security protocols, electronic transmissions may be intercepted by unauthorized parties;
  • Your provider cannot perform a physical examination; your self-reported health information and any submitted lab results constitute the clinical basis for your care;
  • Delays in communication may occur outside of normal business hours.

In the event of a technical failure during a scheduled consultation, your provider or a care coordinator will contact you by telephone to reschedule or continue your visit.

4. Privacy and Confidentiality

All telehealth interactions, health records, and communications are subject to the same confidentiality protections as in-person medical visits under HIPAA (45 C.F.R. Parts 160 and 164) and applicable state law. Concierge Med Partners uses HIPAA-compliant technology platforms and maintains administrative, physical, and technical safeguards to protect your Protected Health Information (PHI). Telehealth sessions will not be recorded without your explicit written consent.

5. Prescribing via Telehealth

Concierge Med Partners' licensed providers may prescribe medications via telehealth following a clinical review of your health intake assessment and, where clinically indicated, laboratory results. Medications prescribed through our platform may include, but are not limited to:

  • Tirzepatide + B12 (GLP-1/GIP dual receptor agonist for weight management);
  • GLO, Glutathione (antioxidant, anti-inflammatory peptide therapy);
  • MOTS-c (mitochondrial-derived metabolic peptide);
  • NAD+ Injections (cellular energy and metabolic optimization);
  • Testosterone Cypionate and related hormone therapy agents (TRT/HRT);
  • Other compounded medications as clinically indicated.

You understand and acknowledge that:

  • A valid provider-patient relationship is established through your intake assessment and provider consultation prior to any prescription being issued;
  • Prescriptions will be transmitted to our licensed compounding pharmacy partners and shipped directly to your address;
  • You are responsible for providing complete and accurate health information, including all current medications, supplements, allergies, and medical conditions;
  • You agree to follow all provider instructions, administer medications only as directed, and promptly report any adverse effects or concerns to your care team;
  • Compounded medications are not FDA-approved finished drug products. They are prepared by licensed compounding pharmacies based on a valid prescription. Risks associated with compounded medications may differ from commercially manufactured drugs.

6. Provider Consultation Fee

An initial provider consultation for clinical review and personalized protocol development is available for a fee of $115.00. This fee covers the licensed provider's clinical time, review of your health history and assessment, lab work recommendations where applicable, and the creation of your individualized treatment plan. This fee will be credited toward your first bundle or treatment purchase.

7. Your Rights as a Telehealth Patient

  • You have the right to withhold or withdraw your consent to receive telehealth services at any time without affecting your right to seek future care;
  • You have the right to request an in-person consultation with an independent provider if you believe telehealth is insufficient for your clinical needs;
  • You have the right to be informed of all persons, including students, trainees, or observers, who are present during your telehealth consultation;
  • You have the right to inspect all records obtained and recorded in the course of your telehealth interactions;
  • You have the right to receive a copy of this Informed Consent at any time by contacting [email protected].

8. Emergency Situations

Telehealth services provided by Concierge Med Partners are NOT appropriate for emergency medical situations. If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, signs of stroke, severe allergic reaction, or any other life-threatening condition, you must call 911 immediately or go to the nearest emergency room. Do not attempt to contact Concierge Med Partners in lieu of emergency services.

9. State Licensing and Geographic Limitations

Our licensed providers practice telehealth in states where they hold active licensure. By submitting your health assessment or booking a provider consultation, you represent that you are physically located in a state where our providers are licensed to provide care. It is your responsibility to notify us if your state of residence or physical location changes, as this may affect your eligibility to receive services.

10. Acknowledgment and Agreement

By using Concierge Med Partners' telehealth services, including submitting a health assessment, booking a provider consultation, or receiving a prescription through our platform, you acknowledge that you have:

  • Read and understood this Informed Consent for Telehealth Services in its entirety;
  • Had the opportunity to ask questions and have those questions answered to your satisfaction;
  • Voluntarily agreed to receive healthcare services via telehealth under the terms described herein;
